  7. Under-taking regarding not taking part in politics.
  8. Consent not regarding recovery of Government dues.
  9. Specimen Signature. Two sets.
  10. Name of Bank and branch with Account Number.
  11. Thumb and finger impression. Two sets.
  12. List of Family members.
  13. Service book along with qualifying service certificate of Non-gazetted/ gazetted period.
  14. History of Gazetted period. (Service Statement).
  15. Last Pay Certificate.
  16. NOC from Estate Office.
  17. NOC from Administrative Department (out side Quetta).
  18. Office Order for retirement from administrative Department.
  19. Descriptive roll (mark of identification etc.)
  20. Option for 50% commutation.
  21. Death Certificate if required.
  22. Non-remarriage certificate if required.
